The $90 Medicare Payment: Who Gets It, and How Far It Goes Against the Part B Premium

More than 20 million people on traditional Medicare get a one-time $90 this week, paid from a $2 billion fund no administration had touched. It covers 42% of this year’s Part B increase, and the payout uses about 90% of the fund.

$90 is a little less than half of one month’s Medicare Part B premium. That is the size of the one-time payment President Donald Trump announced on Friday, 2 October, for more than 20 million people on Medicare, and it is landing in bank accounts this week.

The White House says the money comes from the Medicare Improvement Fund, a pot Congress set up in 2008 and gave $2 billion, which no administration had used before. Most payments go out by direct deposit in early October, with cheques by post for people without one.

Who gets the $90, and who doesn’t

According to the White House fact sheet, “most Medicare Part B enrollees are eligible.” Two groups are excluded:

  • people whose Part B premiums are already paid by Medicaid;
  • people who pay an Income-Related Monthly Adjustment Amount (IRMAA), the higher premium charged above an income threshold.

CBS News reports two further exclusions: people enrolled in Medicare Advantage plans, and beneficiaries living outside the United States. The fact sheet itself doesn’t mention Medicare Advantage, but the Medicare Improvement Fund was created for the traditional fee-for-service programme, which fits CBS’s reading. CBS puts the number of people left out at about 40 million.

A $2 billion fund, mostly spent in one go

The fact sheet says “more than 20 million” people will be paid. At $90 each, 20 million payments come to $1.8 billion, which is 90% of the $2 billion Congress put in the fund. If the final number of recipients is higher, the share is higher too. Either way, a fund that sat unused for 18 years is being mostly emptied in a single payment, a month before the 3 November midterm elections.

It is the second such payment in a month. On 10 September the administration announced $500 refunds to about 950,000 HealthCare.gov buyers, which began going out on 30 September.

What $90 covers against the premium

For most people the Part B premium is deducted from their Social Security payment before it arrives, so the useful comparison is with how much that deduction has grown. The premium rise and the 2027 cost-of-living adjustment land on the same payment, so the net change is what a recipient sees, not either figure alone.

Sources: CMS, 2026 Medicare Parts A & B premiums fact sheet; 2027 figure is the Medicare Trustees' projection, not a final premium (CMS usually announces it in November). Yearly = monthly × 12; percentages are our calculation.

So the payment covers less than half of this year’s rise, but more than the rise the Medicare Trustees project for next year, if their projection holds. “It won’t fully offset higher costs” facing beneficiaries, Juliette Cubanski of KFF told CBS News. She said people paying the income-related premiums were left out because they “may be better able to absorb higher costs.”

When it arrives, and what to do if it doesn’t

Direct deposits are going to the account Social Security already pays into, on or around 8 October according to CBS News. Paper cheques are mailed to the address registered with Medicare and will arrive later in the month. Nobody needs to apply.

The 2027 Part B premium itself will be announced by CMS in November, after the election. That figure, not the $90, will decide how much of next year’s Social Security raise beneficiaries actually keep.
